Insurance - Our primary concern is your care, however
we would like you to maximize your insurance benefit. We will
provide information as needed or requested to submit forms to
enable you to receive the benefits you deserve. Please make sure
that you provide us with all the necessary and updated information
that we will need.
Payment options – 
1. Cash- 7% Accounting Reduction – Available
only on fees in excess of $300.00 and when paid prior to the
day of treatment.
2. Major Credit Card – VISA, Master Card,
AMEX, Discover, etc.
3. Extended payment plans *
90 Days Same as Cash – Fees in excess
of $300.
12 Months Same as Cash - Fees in excess of $1500
Extended payments plans to 36 months
